11 Month Employee Summer Savings / Arrears Pay Periods Request Form (PDF, 175KB, Fillable PDF)

This form allows 11 month Certificated and Classified employees to opt in, change, or opt out of the Summer savings program. This program splits eleven (11) normal paychecks over twelve (12) checks.

12 Payments – Summer Savings Program (Summer Arrears).

11 months of pay paid over 12 months

Paychecks issued August to June of each year

July Summer check issued on 06/30 of each year

Employees and Substitute Employees Direct Deposit Authorization Form (PDF, 264KB, Fillable PDF)

Must be submitted by the employee in person with a voided check or bank letter.

Instructions are contained in the form.

No forms will be accepted or authorized through email as a fraud prevention.

All checks will be mailed on payday for employees without direct deposit.

Employees with updates to their direct deposit or bank changes will be contacted by Payroll for a one-time pickup while the change occurs.

New employees will be offered a pickup time for their first check.
